*In cooperation with local, state and federal public health departments, Park Avenue Healthcare & Wellness Center will test all residents and staff members for COVID-19 as recommended. All COVID-19 tests performed at Park Avenue Healthcare & Wellness Center are through a lab approved by the California Department of Public Health (CDPH).
